ABSTRACT

La encefalitis por virus herpes simple (VHS) es una causa frecuente de encefalitis grave y potencialmente fatal. La encefalitis autoinmune posherpética (EAPH) afecta a un porcentaje de los pacientes que han presentado encefalitis herpética (EH) y se caracteriza por la aparición de nuevos síntomas neurológico/psiquiátricos, y/o por el empeoramiento de los déficits adquiridos durante la infección viral dentro de un lapso temporal predecible. Se produce por un mecanismo no relacionado con el VHS, sino por fenómenos autoinmunes, y es susceptible de tratamiento con inmunomoduladores. Se presenta el caso de un varón de 5 años de edad con EAPH que requirió tratamiento inmunomodulador, de primera y segunda línea, con buena evolución y remisión de los síntomas.


Herpes simplex virus (HSV) encephalitis is a common cause of severe and potentially fatal encephalitis. Autoimmune post-herpes simplex encephalitis (AIPHSE) affects a percentage of patients who developed herpes simplex encephalitis (HSE) and is characterized by the onset of new neurological/psychiatric symptoms and/or worsening of deficits acquired during the herpes infection within a predictable time frame. It is caused by a mechanism not related to HSV, but by autoimmune conditions, and is susceptible to treatment with immunomodulators. Here we describe the case of a 5-year-old boy with AIPHSE who required first- and second-line immunomodulatory treatment, with an adequate course and remission of symptoms.

ABSTRACT

O mieloma múltiplo é a segunda neoplasia onco-hematológica mais comum, correspondendo a 1% das neoplasias malignas. Trata-se de uma condição subdiagnosticada, assim como a encefalite herpética. Entretanto, é comum a relação do mieloma com infecções, em decorrência do acometimento da imunidade humoral. A encefalite viral herpética tem como principal agente o vírus do herpes simples 1. O caso foi descrito baseado em um raciocínio clínico, visando contribuir para uma melhor caracterização do quadro clínico e do diagnóstico de duas entidades clínicas que possuem baixa suspeição diagnóstica e elevada morbimortalidade. Durante o curso da investigação, foram propostas variadas etiologias como responsáveis pelo rebaixamento do nível de consciência, levando a uma pesquisa de um espectro mais amplo de hipóteses diagnósticas, as quais precederam a confirmação do diagnóstico final. O quadro clínico atípico descrito apresentou obstáculos à suspeição diagnóstica correta, pois a ausência dos sinais e sintomas característicos de ambas as doenças levaram à pesquisa de um arsenal mais amplo de diagnósticos diferenciais. Assim, o atraso no diagnóstico e o início tardio do tratamento foram fatores que contribuíram para o prognóstico reservado do paciente. A associação dessas doenças é pouco descrita na literatura, de modo que mais estudos acerca do assunto se fazem necessários. (AU)


Multiple myeloma is the second most common onco-hematologic neoplasm, accounting for 1% of malignant neoplasms. As herpetic encephalitis, it is an underdiagnosed condition. However, the relation of myeloma with infections is common, due to the involvement of humoral immunity. Herpetic encephalitis has as its main etiological agent the herpes simplex virus 1. The case was described based on a clinical reasoning, aiming to contribute to a better characterization of the clinical picture and diagnosis of two entities that have low rates of diagnostic suspicion and high rates of morbidity and mortality. During the course of investigation, several etiologies were proposed as responsible for the decreased level of consciousness, leading to a search for a broader range of diagnostic hypotheses, which preceded confirmation of the final diagnosis. The atypical clinical picture described presented obstacles to the correct diagnostic suspicion, as the absence of symptoms and signs characteristic of both diseases led to the search for a broader arsenal of differential diagnoses. Thus, delayed diagnosis and late treatment were factors that contributed to the patient's reserved prognosis. The association of these diseases is poorly described in the literature, so further studies on that subject are required. (AU)

ABSTRACT

El uso cada vez más difundido de la terapia antirretroviral de gran actividad (TARGA) en el tratamiento de los pacientes con infección por el virus de la inmunodeficiencia humana (VIH) puede dar lugar a respuestas paradojales, caracterizadas por un empeoramiento de las manifestaciones clínicas o la reactivación de ciertas infecciones oportunistas, hasta ese momento subclínicas, como el Herpes varicela-zóster (HVZ). Este cuadro clínico se conoce desde hace años como síndrome inflamatorio de reconstitución inmune (SIRI). Puede afectar a más del 30% de los pacientes seropositivos para el VIH con un tiempo de aparición promedio de 8 a 12 semanas luego del inicio o cambio de TARGA. El HZ mucocutáneo representa entre el 7% al 12% de los episodios de SIRI en estos pacientes. En este trabajo, se presenta un paciente VIH seropositivo que desarrolló un episodio de HVZ cutáneo monometamérico asociado a compromiso del sistema nervioso central bajo la forma de un síndrome meningoencefalítico


The use of highly active antiretroviral therapy (HAART) in the management of human immunodeficiency virus (HIV) infection has resulted in a paradoxical response associated with the worsening of clinical symptoms of previously subclinical infections, such as herpes varicella-zoster (HVZ). This clinical picture is named as immune reconstitution inflammatory syndrome (IRIS). It may affect up to 30% of HIV-seropositive subjects within a wide range of time after the initiation or change of HAART, but mainly after 8 to 12 weeks. Mucocutaneous HZ accounts for 7%-12% of the diseases associated with HIV infection in patients with immune reconstitution from the administration of HAART. Here we present an HIV seropositive patient that developed an episode of cutaneous metameric eruption of HVZ associated with central nervous system involvement as meningoencephalitis syndrome.

Introduction: Herpes simplex virus (HSV) is the most common etiology of sporadic encephalitis and presents with an estimated mortality of 50-70%. Objective: To describe baseline characteristics of patients with herpetic encephalitis admitted to a tertiary teaching hospital and their difference with patients with non herpetic encephalitis. Materials and Methods: Nested case control study using a retrospective cohort of patients with suspected encephalitis admitted to the Hospital Italiano de Buenos Aires (2006-2013). Adult patients included had a lumbar puncture with a positive or negative polimerase reaction for HSV. A case of herpetic encephalitis was defined as a positive polimerase reaction in spinal fluid. For each case, 5 controls were randomly selected. Results: There were no baseline differences present between cases and controls. The only covariate associated with herpetic encephalitis was an abnormal brain magnetic resonance imaging (MRI) (OR: 5.37, IC 95% 1.42-20.38, p < 0.01). The most frecuent alterations in the MRI were extratemporal lesions or temporal ones with or without haemorrhage. Discussion: There are no apparent baseline clinical differences between patients with or without herpetic encephalitis. A positive finding in a brain MRI should be taken into account during clinical workup.


Introducción: El virus herpes simplex (VHS) es la causa reportada más común de encefalitis esporádica con una mortalidad estimada de 50 a 70%. Objetivo: Describir las características de los pacientes con encefalitis herpética (EH) en nuestro medio y sus variables clínicas asociadas. Materiales y Métodos: Estudio anidado de casos y controles sobre una cohorte retrospectiva en el Hospital Italiano de Buenos Aires (2006-2013). Se incluyeron pacientes adultos con sospecha de encefalitis a quienes se les realizó una punción lumbar con posterior reacción de polimerasa en cadena para VHS en líquido cefalorraquídeo. Por cada caso (reacción positiva para VHS) se tomaron cinco controles aleatoriamente seleccionados. Resultados: No se observaron diferencias significativas en las características demográficas y clínicas entre los casos de EH y los controles. La única variable clínica asociada al diagnóstico de EH fue la alteración en la resonancia magnética (RM) de cerebro (OR: 5,37, IC 95% 1,4220,38; p < 0,01). Los patrones más comunes de alteración en la RM fueron las lesiones extra-temporales o lesiones temporales con o sin hemorragia. Discusión: Este hallazgo nos hace jerarquizar el hallazgo de un resultado positivo en la RM durante la valoración inicial de un paciente con clínica compatible de EH.

ABSTRACT

Herpes simplex encephalitis is a diagnostic challenge and causes high morbidity and mortality in children. Early suspicion of the disease and a rapid, safe and useful diagnostic test are relevant because up to 70% of the cases may die. We report the case of a newborn girl aged 25 days, who presented with a clinical picture that was compatible with herpes simplex encephalitis where the confirmation of the etiological diagnosis was delayed. Only by repeated real-time polymerase chain reaction it was possible to confirm the presence of herpes simplex virus type 1 in the cerebrospinal fluid.


La encefalitis herpética genera un desafío diagnóstico y es causa de alta morbi-mortalidad en niños. Se requiere de una sospecha clínica precoz y una prueba diagnóstica útil, rápida y segura, ya que sin tratamiento oportuno y adecuado, hasta 70% de los casos puede fallecer. Comunicamos el caso de una recién nacida de 25 días de vida, que presenta un cuadro clínico compatible con encefalitis herpética, donde el diagnóstico etiológico tardó en ser confirmado y sólo la técnica de reacción de la polimerasa en cadena en tiempo real (RPC-TR) aplicada de forma repetida permitió certificar la presencia de virus herpes simplex tipo 1 en el LCR.

CONTEXT: Meningoencephalitis early in life, of any etiology, is a risk factor for development of subsequent sequelae, which may be of physical, psychiatric, behavioral or cognitive origin. Anomia is a language abnormality frequently found in such cases, and other language deficits are rarely described. The aim of this study was to describe the cognitive and linguistic manifestations following a case of herpetic meningoencephalitis in a 13-year-old patient with eight years of schooling. CASE REPORT: The patient underwent a speech-language audiology assessment nine months after the neurological diagnosis. The battery of tests included the Montreal-Toulouse Language Assessment test protocol (MT Beta-86, modified), the description from the Cookie Theft task of the Boston Diagnostic Aphasia Examination (BDAE), an informal assessment of the patient's logical and mathematical reasoning, and the neuropsychological subtests from the WAIS-III scale, which assess working memory. The patient presented mixed aphasia, impairment of short-term memory and working memory, and dyscalculia. This case also presented severe cognitive and linguistic deficits. Prompt diagnosis is crucial, in order to enable timely treatment and rehabilitation of this neurological infection and minimize the cognitive deficits caused by the disease.


CONTEXTO: Meningoencefalite em idade precoce, de qualquer etiologia, é fator de risco para o desenvolvimento de sequelas posteriores, que podem ser de origem física, psíquica, comportamental ou cognitiva. Anomia é uma manifestação linguística frequente nesses casos e outras alterações de linguagem raramente são descritas. O objetivo do estudo foi descrever as manifestações linguísticas e cognitivas pós-meningoencefalite herpética de um paciente com 13 anos de idade e oito anos de escolaridade. RELATO DE CASO: O paciente passou por avaliação fonoaudiológica nove meses após o diagnóstico neurológico. A bateria de avaliação incluiu o Protocolo de Avaliação de Linguagem Montreal Toulouse (MT Beta-86 modificado), a descrição da prancha Roubo dos Biscoitos do Teste de Boston para o diagnóstico da afasia, avaliação informal do raciocínio lógico-matemático e os subtestes neuropsicológicos da escala WAIS-III que avaliam a memória de trabalho. O paciente apresentou afasia mista, alteração da memória de curto prazo e da memória de trabalho e discalculia. Apresentou também alterações cognitivas e linguísticas graves. O diagnóstico precoce tem fundamental importância para o tratamento oportuno e a reabilitação dessa infecção neurológica e também para minimizar as alterações cognitivas causadas pela doença.

ABSTRACT

Diagnosis of herpes simplex encephalitis (HSE) is based on the detection of herpes simplex virus (HSV) DNA in patients' CSF samples. HSV DNA quantitation has the potential for estimating the effects of antiviral therapy. The aim of this study was to diagnose HSV DNA in HSE suspected patients and the quantitative analysis of its genome using real-time PCR to assess the value of the viral load in the course of antiviral treatment. The CSF samples were collected from 236 consecutive HSE suspected patients from November 2004 to May 2008. Upon DNA extraction, the samples were analyzed by Real-Time PCR assay. A set of primers amplified a common sequence of HSV glycoprotein B gene. The copy numbers of unknown samples were expressed via a standard curve drawn with a known amount of amplified cloned plasmid. Of the 236 samples, 137 (58 percent) came from males and 99 (42 percent) from females. The HSV genome was detected in 22 (9.3 percent) patients by PCR, 13 males/ 9 females. Serial CSF samples were available from 10 of the 22 patients. The range of the HSV DNA copy numbers in the clinical samples ranged from 2.5 × 10² to 1.7 × 10(6) copies/mL of CSF. Quantitative PCR results can be helpful in evaluating the efficacy of antiviral therapy in the above-mentioned patients. There is an association between the initial viral load and the duration of treatment course.

ABSTRACT

Herpes simplex encephalitis is an infrequent infection with high mortality and morbidity. Antiviral therapies decrease mortality but long term sequelae are still high, so early diagnosis is important for opportune treatment. We present a pair of twins with central nervous system herpes simplex infection during the first month of life. Both twins presented non-specific symptoms and consulted with 48 hours apart needing intensive care admission, the first one for noninvasive mechanical ventilation and the second for hemodynamic support. Diagnosis was made by cerebrospinal fluid PCR, in the first twin at day 9 of disease and in the second at admission. Both twins were treated with acyclovir, but only the second one at the beginning of her illness. Initial study with electroencephalogram and magnetic resonance was normal and cerebrospinal fluid on day 18 of treatment was negative for herpes simplex virus DNA detection in both patients.


La encefalitis herpética en una infección poco frecuente, pero que condiciona alta morbilidad y mortalidad. Las terapias antivirales han logrado disminuir la mortalidad pero no las secuelas a largo plazo que siguen siendo altas, por lo que el énfasis está puesto en la precocidad del diagnóstico, en aras de implementar un tratamiento oportuno. Se presenta el caso de dos gemelas con encefalitis causada por virus herpes simplex durante el primer mes de vida. Ambas gemelas presentaron síntomas inespecíficos al mes de vida y consultaron con 48 horas de diferencia, necesitando cuidados intensivos, la primera por requerimientos de ventilación mecánica no invasora y la segunda por inestabilidad hemodinámica. El diagnostico fue realizado por RPC cualitativa en LCR positivo para VHS, en la primera gemela el día 9 de síntomas y en la segunda al momento de su consulta. Ambas gemelas recibieron aciclovir, pero sólo la segunda precozmente, desde el inicio de los síntomas. El estudio inicial en ambas, incluyendo EEG y RM, resultó normal y el LCR del día 18 de tratamiento no presentaba ADN de VHS en ambas pacientes.

ABSTRACT

Herpes simplex encephalitis (HSE) is a leading cause of sporadic, nonepidemic viral encephalitis in children and adults. We report a very rare case of HSE with involvement of bilateral thalamus, putamen, upper pons and midbrain, with development of extrapyramidal symptoms which responded to corticosteroid therapy. A 15-mth-old female baby admitted with complaint of fever for 5 days and generalised tonic clonic seizure 10 hours before admission. On clinical examination patient was drowsy, temperature was 39.4 oC and vitals were stable with signs of increased intracranial tension. There were no signs of meningeal irritation. Patient gradually become unconscious in the next few hours and pupils were constricted bilaterally with development of atonia in all four limbs and neck muscles. Doll’s eye phenomenon was absent.

ABSTRACT

The high signal intensities in bilateral mesiotemporal lobes on T2-weighted images are typical findings of herpes encephalitis or paraneoplastic limbic encephalitis. We report a case of neurosyphilis with mesiotemporal involvement on MRI. Positive antibodies in the cerebrospinal fluid confirmed the diagnosis. The results suggest that neurosyphilis should be considered when MRI results indicate mesiotemporal abnormalities.

A encefalite herpética (EHS) é uma patologia grave, com alto índice de morbidade e letalidade. Esta doença se expressa por um quadro clínico agudo, tendo como principais manifestações:febre, cefaléia e alterações cognitivas e psíquicas. Desta forma, a suspeição clínica, associada a exames laboratoriaise de imagem são de fundamental importância para a detecção precoce e tratamento imediato desta patologia, a fim de impedir sua progressão rapidamente fatal. The herpes simplex encephalitis (HSE) is a serious pathology, with a high rate of morbidly and lethality. This diseasepresents a acute clinical condition, it had like principal symptons: fever, headache and cognitive and psychological alterations. In such a way, the clinical suspicion is associated to laboratorials examinations and of image. This are extremely important to early detect and fast treatment this pathology, on purpose to impede the quickly fatal progression of the illness. A encefalite herpética é defenida como um processo inflamatório agudo do parênquima encefálico em conseqüência da infecção pelo vírus herpes simples. O presente trabalho refere-se a um caso de encefalite herpética em lactente de um ano e oito meses de idade após quadro de infecção das vias aéreas superiores, que evolui com atraso no desenvolvimento neuropsicomotor. O objetivo do estudo é demonstrar a necessidade do diagnóstico precoce, em que o uso de drogas antivirais específicas contribui para o prognóstico do paciente.

ABSTRACT

São descritos sete surtos e um caso isolado de meningoencefalite por herpesvírus bovino-5 (BoHV-5) em bovinos no Rio Grande do Sul entre 2002 e 2004. Foram afetados bovinos de 1-18 meses, de diversas raças e ambos os sexos. A maior freqüência foi observada em bovinos recém-desmamados e submetidos a outros fatores de estresse. Nesses surtos, de uma população total sob risco de 1.359 bovinos, 54 foram afetados, quatro se recuperaram e 50 morreram espontaneamente ou foram submetidos à eutanásia quando moribundos. Os índices gerais de morbidade, mortalidade e letalidade foram, respectivamente, de 3,97 por cento, 3,67 por cento e 92,59 por cento. A evolução clínica variou de 3-10 dias e os sinais eram caracterizados por depressão, corrimento nasal ou ocular, ranger de dentes, andar em círculos, cegueira, febre, nistagmo, tremores, anorexia, disfagia, sialorréia, incoordenação, pressão da cabeça contra objetos, pêlos arrepiados, taquicardia, taquipnéia, dor abdominal, melena, quedas, decúbito, opistótono, convulsões e movimentos de pedalagem. Dezenove bezerros foram necropsiados. Achados de necropsia foram caracterizados por hiperemia das leptomeninges, tumefação das porções rostrais do telencéfalo, com achatamento das circunvoluções dos lobos frontais; nessas áreas havia focos marrom-amarelados e amolecidos (malacia). Nos casos de evolução clínica mais longa era observada acentuada tumefação, amolecimento e extensas áreas de hemorragia nos lobos frontais telencefálicos. Microscopicamente, todos os bovinos afetados apresentaram meningoencefalite não-supurativa e necrosante, que variou quanto à localização e intensidade nos 19 casos examinados e nas seções de encéfalo de um mesmo caso.


Seven outbreaks and an isolated case of meningoencephalitis caused by bovine herpesvirus-5 (BoHV-5) in cattle in Rio Grande do Sul, Brazil, occurring in 2002-2004, are described. From a total population at risk of 1,359 cattle, 54 1-18-month-old calves from both sexes and several breeds were affected and 50 died spontaneously or were euthanatized while moribund. The highest frequency of cases was in recently weaned calves or calves submitted to other stressing factors. General rates of morbidity, mortality and lethality were respectively 3.97, 3.67 and 92.59. Clinical courses varied from 3-10 days and included depression, nasal and ocular discharge, grinding of teeth, circling, blindness, fever, nistagmus, trembling, anorexia, dysphagia, drooling, incoordination, head pressing, rough hair coat, tachycardia, tachypnea, abdominal pain, melena, falls, recumbency, opisthotonus, convulsions and paddling. Nineteen calves were necropsied. Necropsy findings were characterized by hyperemia of leptomeninges, swollen rostral portions of the telencephalon, and flattening of frontal lobes gyri; frequently in these frontal areas there were segmental brown-yellow discoloration and softening (malacia) of the cortex. In cases with more protracted clinical courses there were extensive swelling, softening and hemorrhaging of the telencephalic frontal lobes. Microscopically, all affected cattle had a necrotizing non-suppurative meningoencephalitis with variable distribution among the 19 cases and among the various telencephalic regions of the same case. The severity of these changes were more marked, in decreasing order of intensity, in the telencephalic frontal cortex, basal ganglia (nuclei), thalamus, brain stem, parietal telencephalic cortex, occipital telencephalic cortex and cerebellum. ABSTRACT

La encefalitis herpética es la causa más frecuente de encefalitis esporádica en el mundo occidental. Para conocer las principales características clínicas de esta enfermedad en nuestro medio, se efectuó un análisis de casos confirmados por reacción de polimerasa en cadena en dos hospitales universitarios de Santiago. Un total de 15 casos pudo ser identificado con un promedio de edad de 41 años (5-78 años) y 80% ³ 30 años, el primero de ellos el año 1998. La mayor parte se presentó con fiebre y compromiso de conciencia (80% cada uno) o cefalea (67%). Las convulsiones y la focalización fueron infrecuentes (£ 15%) y sólo 2 casos (13%) tuvieron además una manifestación herpética extracerebral. La duración promedio de los síntomas fue de 3,8 días. La mayor parte estuvo asociada al serotipo 1 (86,7%). El 91,7% de los casos evaluados presentó alteraciones electroencefalográficas, 81,8% alteraciones en la resonancia magnética y sólo 13,3% en la tomografía axial computarizada. La totalidad de los pacientes fue tratada con aciclovir y la letalidad fue de 13,3%, aunque el deceso en un paciente con SIDA y linfoma del SNC no pudo ser atribuido a la infección herpética. Seis pacientes (40%) presentaban secuelas neurológicas al momento del egreso. La muerte o alteraciones neurológicas al alta estuvieron asociadas significativamente a un inicio del tratamiento > 3 días desde el inicio de los síntomas. (p = 0,01 prueba bilateral de Fisher).

ABSTRACT

Encephalitis due to herpes simplex virus [HSV] is the most important cause of fatal Sporadic encephalitis that prompt diagnosis is necessary because there is effective antiviral treatment for this disorder. In the fatal cases, severe meningoencephalitis is associated with diffuse destructive changes in the brain tissue and is following the reactivation of latent virus.Recent studies demonstrate increased hypercoagulopathy state due to HSV infection in the endothelium and footprints of virus has been found in the vascular thrombosis. In this issue, one of the very rare manifestations of HSV encephalitis, which has so far been reported in a neonate, is presented. The patient presented with the signs of encephalitis including stupor state and agitation, headache, seizure and neck stiffness. The diagnosis of HSV encephalitis is confirmed by the PCR test of CSF fluid .MRI and MR venography of the patient's brain demonstrated the thrombosis of sagital, rectus and lateral venous sinus. The patient survived with the specific treatments. Herpes simplex virus type 1 can be a cause for the development of cerebral Venus thrombosis. This case report is a conformation for the studies demonstrating hypercoagulopathy state in the setting of HSV infection
